I don't know exactly but if it does not start then how the fuel will be wasted?? :)
It is not so much hard for a car to start when the engine is cold as it is easier for a car to start when the engine is warm.Cars that are hard to start in the cold are usually not new cars any more and because of wear and tear there is more likely to be something that doesnt work as well. when fuel is warm it ignites more easily.

Yes, That is why new cars are coming with a stop-start feature. I the engine will shut off if the car is stationary for a given amount of time( to improve mpg).
The heater in your car uses waste heat from the engine to heat the car. When the engine is not running there is no heat to distribute.
You start the engine in neutral.
