Yes it does but you will need the newwer style fog lamps too as 98 won't fit in a 2003 front bumper.

I wasn't aware there is a 1996 model of Malibu, but on my 1998, the horn is behind the passenger side front fender, between the bumper and wheel. To access it, you must remove the entire front bumper cover.

There is a difference. The 1992 front bumper has a more rounded look than the 1991 model but it should fit. Not 100% sure about that but I suspect they should both bolt on the same.
No, the 66 model is different then the 67 model. I am not shure if the 65 model would fit - it is possible.
You don't say what Lincoln, but it is unlikrly, F150 is a pickup truck and would have a very different bumper than any model lincoln
