I'd just tell whoever you feel most comfortable with (Parent(s), friend, nurse ect.)
Then, if they don't know, I would seriously advise you to get a checkup from a doctor or the school nurse.
In school, make up like a signal with your mate like coughing or something, if your feeling light headed, ask your teacher to go to the toilet and if you don't come back, your mate will know you've passed out again and hopefully ask the teacher if they can checkup on you.
