Just a couple of things to check. Check the emergency brake is off. Make sure your fluid is full by checking on flat ground while the engine is running after you ran through all the gears. You might want to flush the transmission and change the two filters. These answers don't sound helpfult but they worked for me. Saved myself a lot of money.

Everything points to the transmission being rebuilt or replaced. The reverse band broke, came loose etc.
The A4LD transmission is the weakest part of the 91-94 explorer. It is likely that your transmission has failed.
