If its an automatic then your either in winter or a low gear(not flashing) or transmission fault(flashing arrow) But if its manual the non flashing arrow is the recommended time to up shift.

MID 128 PID 111 has to do with the engine coolant level in the Volvo. It is a code that can be gotten when the check engine light is flashing.
the relay needs to be replaced. you will find it behind the hazard switch (attached to the switch - a long skinny thing) I looked forever till someone told me where it was. Even the Volvo parts guy didn't know and Lawrence Volvo of Walnut Creek, CA would not tell me...
