A revoked license is completely canceled and the individual no longer has driving privileges. A restricted license, on the other hand, allows driving under certain conditions or limitations, such as only for work or medical appointments.
It is not possible to put a lien on a person's driver's license in any state. A driver's license can be suspended or revoked but only for traffic violations.
In some states a provisional license is given to new drivers, restricting them to what hours and where they can drive. A form of provisional is also given sometimes to someone who has a revoked license, allowing them to drive for working duties.
There are some companies that will write a policy if you have a suspended license as long as it is not permanently revoked.

Yes. A license cannot be 'loaned' to someone else.

No, but you should be able to get an international driving permit that would carry some weight. In the US, US citizens can get them at AAA. International driving permits are not good as license substitutes in the country where they were issued. In other words, someone in the US can't get and use one in place of a US license that has been suspended or revoked.
