Click on the settings icon on the top left corner of google chrome.This will open settings window, then click on the Extensions link on the left to see all extensions that are installed on your chrome.Find the extension that you want to remove and click on the Delete(garbage bin) icon to to permanentely remove the extension from chrome. Here you can also disable an extension instead of permanentely deleting it by unchecking the ENABLED icon next to it.
