You either have to inspect the fecal matter in the hen house or look at the cloaca of each hen for worm sign. Diatomaceous earth added to the chicken feed will kill most common worm infestations.

If you mean tapeworm, it will be pretty obvious as they will throw up more often or have a lot of head aches through out the months and years and followed by diarrhea
Not to mention the incredible weight loss and constant urge to eat something as their metabolism has now increased by 4 folds.. they will appear skinny and no matter what you give them they will always remain skinny because the tapeworms are feeding off of the nutrients
