Wilson had a stroke during his second term which disabled him . His wife protected him and the exact extent of disability was kept hidden . This was before the 25th amendment was ratified, so there was no way a disabled president could temporarily be replaced. The only option if he were truly unable to carry out his duties would have been to impeach him and remove him from office.
