A virgin is someone who has not had sex, it's not a physical state of being - when you have sex your vagina is not changed in any way so there is no way to tell just by looking if you are a virgin or not. Using tampons makes no difference either, sometimes vaginal penetration can cause the hymen to tear but usually only if what you're inserting is large and force is used.
The hymen can tear long before you have sex as a result of using tampons, menstrual cups, masturbation, sex toys, speculums, fingers, or as a result of rape. Equally the hymen can remain in tact long after you start having sex as the hymen doesn't cover much of the vaginal opening and is flexible so can accommodate fingers, toys, or penises without tearing (remember sex does not just mean a penis going into the vagina). The hymen is not an indication of virginity.
You are still a virgin if you use a tampon but I don't really know how doctors check but I think there's a piece of skin 'down there' that breaks when you first have 'stuff'

As long as the user is relaxed and using tampons correctly they will simply insert the tampon and it will absorb flow just like it's supposed to. It's no different to any other time they use tampons.
