In a letter, I would address an Imam as:
Dear respected Imam John Dow,
some address an Imam as Sheikh, very common among Arabs in the Middle East,
Dear respected Sheikh John Dow,
and instead of saying Mr., people would say Sheikh, or Imam:
Imam Dow has been kind enough to accept our invitation.
Sheikh Dow has been kind enough to accept our invitation.

The return address on a letter is the address of the person sending the letter. It is important for the recipient of the letter to know from whom and where the letter originated and to have an address to respond, if necessary.
In English you call him: leader of the mosque. But in Arabic you call him: Imam ul-masjid. However, most people just call him : Imam.
