Well, you would first start out analyzing what the gap is. Then, I would actually take a lesson / read a book about the missed topic / etc. Afterwards, I would actually try to apply the knowledge that I was missing (and had just learned).
For example, if I took Algebra I in high school, and then I did not take Geometry, and now I need to go into Algebra II.
I would go to a Geometry class or look up the most important concepts of Geometry. When I am confident that I understand the important stuff, I would actually try to solve some problems from the Algebra II book, to sort of apply what I have learned. If I am still struggling to understand what is going on, I would just touch up on the areas where I am weak.
There are a lot of ways about how people go to solve problems like these, but this path makes most sense to me. I am not a teacher or anything, though.
