You can replace the word Sincerely with some other correct word. However, that simple word is correct and current. The punctuation has also simplified to the commas as in this example. You do not use Mr., any other honorific, or the prime minister's last name.

"Prime Minister" is not a recognised title in the United Kingdom, therefore addressing the current Prime Minister as "Prime Minister David Cameron" would be incorrect. However the title "The Right Honourable" can be used, such as The Right Honourable David Cameron MP, Prime Minister of the United Kingdom.
