In the old days when dinosaurs roamed the Earth (before about 1994), you would write to their admissions office and request an application form, then fill it out. Or, if you're currently in high school and it's a local college, talk to your guidance counselor, who probably has some already.
Nowadays, the college almost certainly has a website where you can download the application form. You may even be able to submit the application online. Be sure and read the instructions, because you may also need to have your high school send a transcript, or possibly have teachers submit references for you to complete the application. You might also have to have your standardized test results sent to the college.

You can transfer to a four year college or university and apply all usable credits toward the bachelor's degree.
It is possible depending on the college or university you apply to. Still, if you do not meet the entrance requirements, then start at a community college. After completion you can then transfer to a four year institution and finish the last two years for the bachelor's degree. The community college system is a great place to start.
Depends on the college. Some look at all four years together, some colleges (I'm pretty sure Stanford is one example) drop your freshman year when calculating GPA.

You do not have to wait four years to reapply. If this were to happen, then find out why you may not have been accepted so you know what to work on. Do not stay out of college. If you do not meet the requirements of the four year college or university, you can start with your home county community college and work on the prerequisites for the other schools. The worse thing you can do it stop your education.
