Raise the rear hatch, and whichever side is bad, go to that side. Remove cover and unscrew housing, unscrew bulb holder, insert bulb and reinstall.
Major Correction here. On a 2003 T&C you raise the rear hatch, look for two black plastic re-tainers. Pop those loose. Carefull to keep hands on tail light lens or it will swing out possibly damaging, light, and or paint. Disconect plug by pulling out red retainer clip. Gently remove plug. take lens to work area. Remove appropriate cap replave bulb.
