take off the tires then take off the casing that holds the brake pads(there are 2 bolts on the back side with a star bolt socket pattern needed to get it off) then the rotor will just slide right off.

front or rear? both are very similar. take the wheels off. loosen and take off the calipers (should be 2 bolts if my memory serves me correctly, the bolts are kinda long. slide the caliper off revealing the brake pads. replace the brake pads and put it all back together. Don't forget to use brake lube on the sliders of the calipers.

Usually it's low brake fluid or a problem with the abs. Check the brake fluid reservoir. Often the brake fluid will go down as the brake pads wear and the calipers has to travel further.
