Look at the sides of the housing and see if it's got a couple of small plastic plugs in it. If so, you need to take a small sctewdriver and gently push the center of these inward. Next, you have to pry these out and then the cover will pull off. There's a couple of bolts that you'll have to remove next. Then you can pull the whole assembly around to face you and access the bulbs.
If you are only replacing the bulb, it can be accessed on the back of the high mount housing in the trunk area.

the light bulb is accessed from the under side of the car bulb is removed by turning the socket 1/4 turn counter clockwise If you are talking about the 3rd brakelight, Unscrew the brakelight mount and turn it so it is easy to unscrew the bulb first. Then unscrew the bulb, it may take a few tries to get it out. If you still have problems unscrew the top of the mount (they're the only screws left on the mount) then try to remove the bulb.
Just take the screws out and the whole assembly will come off the van. Unplug wiring harness. Remove bulb holder and replace bulb.
Open the hood, look behind the head light that is out. There is a round plastic piece with a couple of wires going in to it. Turn the piece counter clock wise and pull the bulb out of the socket. When you mount the new bulb in its place, use a glove or clean clothe to mount the new bulb. If you use your bare fingers/hands, you could leave oil from you hands on the bulb. The oil from your hands or fingers on the bulb could possibly cause the bulb to over heat and burn out the bulb. I believe it says on the package not to use your hands with out gloves. Just mount every thing in reverse going clock wise to securely mount the new bulb socket.

Remove the back light cover housing by removing 2 Philips screws which mount it to the frame. Pull the wiring harness out of the where the bulb sits. Remove the old bulb from the clip and replace.
