Depending on the model and year of the Lexus, you may or may not be able to change the rubber insert. I recommend replacing the entire wiper blade just for extra safety. See sources and related links below for wiper blade options and information.
When you seal your blade it prevents splinters from coming off of your blade when you change the rubber, and it makes it easier to change the rubber. It is protection. When you get a new blade it looks all nice and clean right? If you don't seal it, it won't look like that in the future. It is highly recommended. I was lazy and didn't seal my new butterfly blade, I changed the rubber and a three inch long chunk of wood came off of it, I was so angry at myself, take my advice and seal your blade.

I had to replace the whole blade assembly. The rubber blade did not come off. The replacement blade from autozone came with the whole assembly. Cost wasn't much more than just the rubber blade style.
Actually, I believe you mean the wiper rubber blade. It will state at the back of any good replacement blade on how to change it. Most dealers themselves would do it for you. Let us know if this helps.
I'm going with the assumption that this question is asking about what are commonly called the rubber blade refill inserts rather than the wiper blade which is everything attached to the wiper arm; blade and frame. Most likely yes - you can just replace the rubber insert however in most cases I don't recommend it, especially if you live in cold winter weather climates which puts a lot of stress on the entire wiper blade or you live in a climate where you use your wipers quite a bit. The difference in price, mechanical skill (and patience) between installing refill inserts and full wiper blades is relatively small. If you replace the whole wiper blade, its cheap insurance that your blades will perform better when you most need them.

High-Level brake lamp bulb change. Open the hatch back. Remove the two rubber grommets behind the stop lamp. Insert the flat blade of a screwdriver into each hole and press down the spring clip. The light unit will pop out of its seat. Close the hatch and change the 16W wedge fitting bulb. Click the light unit back into place and replace the two rubber grommets. Easy when you know how!
