The fuses are called a "cannon starter kit" and are at the top of the tall ship's mast above the Pirate Outpost. You can climb up the outpost at the left, then cross right to reach the mast from the top of the Cannonry shack. After you get the fuses, continue down the left side and pick up the small barrel of gunpowder. If you push the barrel into the water and float it beneath the Giant Gold Tooth, you have something to shoot at with the left side cannon.

Swim left to the bottom left cannon, climb up, and jump over the hostile pirate. Cross to the right and climb up and above the Cannonry. On the way down the mast, push the small barrel of explosives off the yardarm to the left, and into the water below. Swim it out to just under the giant Golden Tooth. Go left and fire the cannon, trying to hit the barrel. If you do, the blast shakes loose the Golden Tooth and reveals the map piece.

When you first dock, run past the trading post, the skull, and jump on the island over to the right. Then go and jump to the next highest 'layer' of pirate outpost. Run over until you reach the skull. Then jump in the eye cavities until you get to the other side. Jump above the fire and on to the chain. Climb up and you are at the top.
