To fix a seatbelt that won't pull out, make sure it is not twisted. Jerk on the belt gently and let it retract before trying to pull it out again.

first inspect the belt to be sure it's not twisted or somthing is preventing it from retracting. if you are a do it yourself kind of guy you can inspect and lubricate the workings however honda has a lifetime warranty on seat belts. Check to see if your seatbelt will be covered by visiting a local dealer. if it's a safety concern it will be replaced. I had one on my prelude replaced due to its inability to retract.

I had this same problem and when I pulled the belt out as far as it would go and looked around in the recoil I found a quarter stuck in there. Pulled the quarter out with needle-nosed pliers and it works fine now. Looking at where the recoil is positioned it would be fairly easy for something to fall down in there.
Buckle your seatbelt.
My parents have this problem on their 99 Sienna. They got an estimate of an outrageous amount to fix the problem, because it involved disassembling the dash to reach the control for the seatbelt indicator, plus replacing a sensor. They've decided it's not worth it and have learned to ignore the annoying flashing light. Sorry.
