reaver-darkness, in a locker mission. (requiers a code to open the chest)
rokturo-earth, in a soul weaver mission.
illumina-light, in a fountain after blackwyn tomb. (must complete blackwyn tomb in moonridge)
skull crusher blaster-fire, from a finale 13th mission which is (no longer available).

Roktoru(earth)- You get it in ravenloss mission-museum by collecting the 3 horns found in the mission.
Reaver(darkness)- You get it in the first mission in The Locker which is in a chest with a lock and the code is 743. (the chest is next to a sea monkey once you go into the sunken ship)
Skull Crush Blaster(fire)-You get it from a mission in the finale 13th war which is (no longer available).
Illumina(light)-You get her from the fountain in Moonridge after once completing the quest blackwyn tomb, which is also found in Moonridge. Once you finished blackwyn tomb you go to the fountain in the middle of town and you need to tell the fountain its name to unlock the gem which is Illumina.
