If you are trying to create an anonymous survey I would suggest posting it on a blog and then use an anonymous messaging service to acquire the data. You'll have much more success and ideally collect more data. If struggled with this too... it really doesn't matter if you use survey monkey or have a survey or poll on a blog- you still have to assure the survey takers they they are totally anonymous.
