ADVICE: Before going to this step MAKE CERTAIN that you have checked all other possibilities (1) make certain the ignition switch is in the engine RUN position- the windows will work only if the ignition is on. (2) Check the fuse AND the circuit breaker. (3) Check the wiring between the switch and the fuse panel.
Only after you have completed these elementary tests, consider replacing the switch itself.

I have a 93 Honda Civic LX with the same problem. I had everything replaced and nothing worked. I took it to a very good electrician and he figured out the problem for me. Its a connector that connects the Master window switch to the rest of the car windows. Mine had moisture damage so it worked on and off and only with certain windows. But the rest of the window switches operated the windows fine. It should be in the door panel/hidden around the door hinge.

You have to replace the whole assembly, you cannot replce the switch itself. The assembly pops out with a standard screwdriver and is attached to a wiring harness. The assembley sells on ebay for ~$100-$150.00
From my 07 manual:Start the engine. Push down and hold the driver's window switch until the window is fully open.Pull and hold the driver's window switch to close the window completely, then hold the switch for about 2 seconds.
