Could be a Faulty relay.
Could be a blown fuse or a faulty circuit breaker, a faulty relay, a faulty temperature sensor or the fan motor may be at fault.
you might have a faulty fan relay that is stuck closed, allowing fan to run at all times or a faulty coolant temperature sensor. the fan relay contacts have welded shut it needs replacing just pull out the relays one at a time until the fan turns of and replace it alternatively temp sender could be faulty but this is unlikely

more likely to be a faulty coolant sensor or the thermostat is faulty and staying open

It would probably be nice to know what we are working on. I had a similar problem on a 1994 ford escort, and it was actually the relay block where the relay plugs in that was faulty. Hope this helps.
Faulty electrics. Thermostat, relay, fuse, fan motor, short in wiring, poor ground. Unfortunately, you might have to have a professional look at it.
If your air conditioner is on, the fan(s) will most often run steady. If the AC is off (or you have none), it could be that you engine is running too hot for various reasons. What does the temp guage say? As well, it could be a faulty fan relay switch.. or faulty fan switch sensor in the radiator.. or a faulty temp sensor on the engine. Lots of possibilities.
