Typically, it's some valve you either turn or push. Air pressure is what lowers the axle. The control valves (along with pressure adjustment valves) can be located either inside or outside of the cab. In some cases, they're located among the PTO controls.

No. In fact, I've never operated a dump truck (or any other vehicle with a lift axle, for that matter) where the knob to raise and lower the lift axle was not in the cab.

There's really no way of knowing what an "average" dump truck is without knowing statistics of how many single axle, tandem, tri-axle, quad, quint, centipede, and superdump dump trucks are out there - to the best of my knowledge, no such statistics have been compiled. At the company I work for, our tandem axle dumps (with steel dump bodies) weigh between 23,000 and 24,500... the 23,000 lbs. trucks are the Peterbilt 330s, and the 24,500 lb. trucks are the Kenworth T800s with "rock tub" steel bodies, high lift gates, and split gate beds. These are the tare (empty) weights, not the loaded weights.
When to lower the axle on a tri-axle dump?
When you're loaded to the point where your weight will exceed your legal weight as a tandem truck. Honestly, you should probably have it lowered any time you're loaded, since a motor carrier officer who sees a tri axle, quad axle, etc. with a raised lift axle is likely to question it... even if you are legal, it's time wasted and another annoyance to deal with. If you mean when after you get loaded, you do it while the vehicle is not in motion - don't lower your lift axle while the vehicle is moving.
