In the US, you just add some chocolate syrup (Hershey's, Bosco) or Ovaltine to your milk. Some other mixes include Malted Milk flavoring.
A basic recipe combines lemon juice and cholcolate chips with a small amount of vanilla, and this is blended into milk.

No, you cannot freeze chocolate milk to make chocolate. The chocolate syrup and the milk are just being blended together and frozen into a block of chocolate milk. You can't derive the chocolate from the milk, either, so it is impossible.
You use pestel & mortar on a chocolate bar to grind it to chocolate dust, then use the chocolate dust on a bucket of milk and you get chocolate milk.

To make chocolate milk you need chocolate and milk. If you have a liquid chocolate like Hershey's or Nesquik you can just poor the chocolate into the milk and mix. If all you have is solid chocolate, you will need to heat up the milk (slowly!) and mix in the chocolateuntil it's all mixed in. Then you can either drink it hot (hot chocolate or wait for it to cool (chocolate milk).
