I have heard of many people using super glue for this, but I don't have personal experience.
depends on the reason the crown had to be taken off if it was for caries the crown should be replaced if it happened just because the pulp started hurting but with no caries involved he might be able to glue the crown with temporary glue preform the root canal and then glue it again permanently -though he will have to add a `core` to the tooth too.
No you can not because they dont have special glue to do it

It really depend on what the crown is made out of. It also depends on what glue you use. But either way, it will never look perfect.
It's strong enough for 'recreational' purposes. If I want to glue wood permanently, I use TiteBond ll yellow glue.
Adhesive glue sticks to many surfaces and as the carrier dries, it will bond to the surface more permanently

It is most likely that the glue/cement was insufficient to keep the crown in place. You will need to return to your dentist to have it fixed.
Tensol Cement joins all plastics such as perspex together permanently. The glue is applied to the surfaces to be glued and they are pressed together. They should be clamped for 24 hours.
You can get a perm that fits it to the flip that you want, or you could glue it that way but it probably won't work as good :)
