1. With reference to your Salary Account offer dated __________ for opening salary accounts of our employees, we are pleased to inform you about our acceptance of the offer, inclusive of all the terms and conditions mentioned therein.

5. We understand that in case of non-credit of salaries for more than 3 consecutive months, the special features offered under the respective salary schemes mentioned above would stand withdrawn and the account shall be treated as a normal savings account under the Bank's standard charge structure and all charges shall be levied as applicable to normal savings accounts.

6. The terms and conditions shall be communicated to all staff members opening their Salary Account with Axis Bank.

7. A list of active employees will be provided to the Bank by us at a monthly periodicity along with the salary upload details that would allow the Bank to identify the inactive salary accounts and migrate those to the non-salary scheme.

8. In case of any wrong details being provided by us to the Bank in terms of individual employee salary account number / net amount to be credited for the employee, we confirm that the Bank is not liable for any losses caused therein.

9. In case an employee exits the organization, we shall inform the employee to either get the Axis Bank salary account migrated to a non-salary scheme furnishing full KYC documents in the process, or to get the salary account closed post the full and final settlement with us.

10. We also wish to inform the Bank that the following official/s is/are designated and authorized to verify and validate the bonafides of all staff members who are opening their Salary Accounts with Axis Bank. Any changes to this effect would be intimated to the Bank well in advance, for record.
