Slip the ring over your ear to the hole. Then, rotate the ring through. The bead should have small dimples on either side. Put one of the dimples on one side of the ring and press the other dimple into the ring **this will take a few tries**. If the ball is too small for the opening press the ring together to make the opening smalller.

Any tattoo place that does piercings can do it, but I advise against. You will need surgery to put it back to normal once you stretch the lobe.
I assume that you're talking about a captive ball ring. The ball is probably just too loose. Just tighten the ring around the ball by squeezing the ring with pliers. Unless of course you have ring closers. Those are easier to use but pliers do in a pinch. If you don't tighten it, the worst that could happen is that the ball falls out and you lose it. If that happens, you could put a piece of eraser or clay in place of the ball for temporary use.
Its better to get it on which ever ear you don't sleep on or talk on the phone with to make healing easier. I'd put it on the less attractive ear. It'll distract from weird cartilage folds and whatnot. No such thing as a "gay ear".

Yeah. Some tongue ring balls are pretty big and may look tacky on lip rings.

From what I know, have your cartilage already pierced before you try to gage it. I pierced my own, then once it healed i put a 14 gauge through it. It didn't really hurt but pinched a little.
It probably won't but it can. Your piercer should put the ball in there pretty tight but if the ball does pop out the ring could slip out.
