I would use a latex screed. like cement but with a latex binder, add water pour 5- 10mm on to the concrete and it self levels.Dries in about a day.Put a poylthene membrane down first if damp is a problem

It depends on the contractor in Lexington you're looking at and they type of ceramic tile. If you already have the tile it'll be cheaper but the floor arrangement can raise the price.

If it has a timber floor under and vinyl is solid to the floor and not to thick screw down 10 mm ply at 150mm centres. seal ply with pva sealer prior too tilling. If it is a concrete floor it is best to lift vinyl.
