You go into your iTunes if not install then install. Follow the instructions. And when you are finished, you can sign your iPhone into th your apple itunes account. You can use this for applications songs & videos.
Its simple you connect you iphone to you computer it should say this device is not register would you like to and chick yes and it will sync it ... keep in mind you can only do this with a iphone, iPod or ipad
If you have iOS5 which is out today you can slide the Setup as New iPhone slider and it is automatically setup. For earlier versions you need iTunes and a Computer. The question isn't very specific at what to setup.

If you just recently got a new iPhone 4, connect it to iTunes and iTunes will identify it as a new phone. If you have previously synced your iPhone 3G with that computer before, then iTunes should give you the option to sync the new iPhone with a backup from your latest iPhone 3G sync. I had this problem too. I had iTunes installed already, but when I tried to sync my iPhone, nothing came up on my iPhone screen. I quit iTunes, and unplugged my iPhone, then plugged it back in and opened iTunes again.

no you cannot use ipod and iphone apps on itunes, but you can buy the apps on itunes and put them on your ipod and iphone
you can't... but if you are doing this because of your iphone or ipod not syncing, you can update your phone to keep up with the new itunes
