Most do not make any changes to an original birth record for obvious reasons. Vital records are intended to document information. For various reasons a child's biological parent, if known, should be documented at least for genealogical and hereditary health reasons. If the child is adopted or the parent's right are terminated the birth record may be sealed by a court order but the original exists in most cases.
