On most newer models (at least mid nineties to current), all you need to do is remove the wheel(s), remove the two (each) brake caliper retainer bolts (usually torx or Allen head), slide the caliper off and suspend it from something (DON'T let calipers hang by their hoses). Now you should be able to slip the rotor off with little or no trouble (you may need to tap it off with a rubber mallet on occasion). If you are replacing the rotor with a new one, you'll need to compress the caliper piston with a 4" C-clamp (or similar tool) and it may be advisable to scuff some of the ridges off of your old brake pads if you're reusing them.
Installation is the reverse of removal.
