Remove the tire and wheel. Remove the axle end. Remove the axle seal. Remove the wheel bearing. Reverse the process to install the new wheel bearing.

You replace it like you would a windshield.
Jack up the body in front of the rear wheels or use a lift. Remove the rear wheels. Put a jack under the rear axle. Unbolt the bottom of the shocks. Be sure the brake lines do not stretch too far as you lower the axle. You may need to remove a few clips to avoid breaking the lines. I needed to put the jack between the axle and the body to push the axle down far enough. Remove the springs and take them out through the wheel wells. Reverse the process.

Open the rear hatch of your van. There are two black button like protrusions behind the hatch next to the turn signal lamp. Pry these out and remove lens. Replace bulb.
This is extremely easy. Unscrew axle nut, which is held on with 140 lbs of torque, then unbolt 4 bolts holding assembly inplace, slide assembly off axle splines, install in reverse order.
