"Transformation" of DNA is called mutation its a random event and happens in every cell of your body. Mostly they are insignificant, they do not alter the results. But in some cases when very important segments are altered the cell is altered too, this may be cancer or lead the cell to its death.

There are some agents that increase the mutation rate, for example X rays, UV light.
