You use an official looking letterhead.

Then you list who from and who the notice is to and the purpose of the notice.

Next: You divide the letter into four columns. In the first column You put a box. In that box, you put your first reason. In the second column by the box is a line. You place your initials on that line.

You put a line of boxes down the page with each one filled in.

In the third column is another group of boxes for the employee with poor performance to fill in explaining his side of the story. Some may be a mistake on your part such as the schedule says I am supposed to be here at noon and you are after me for not being her at 10 am. Usually such is not the case. Still, it is a check on misunderstanding.

At the bottom is a statement that if he does not improve he will be terminated.

You sign and he either signs or you make a note that he refused to sign.

You give him a copy.

You must confront him (or her) face to face. (If there is no face to face confrontation complete with paperwork, it is not firing for cause, it is a layoff.)
