This happens in a very few places, and the other possibilities are usually listed in footnotes.

Most of the uncertainties in translation involve the identification of animals, plants, minerals, precious stones, objects, locations, and numbers. The original information identifying these items was lost during the 70 year exile in Babylon. Nobody who had seen these items to associate them with the words was still alive when the Jews returned from the exile.

The Bible has not changed. Today's languages are changing, requiring new translations. For example, we do not today speak the language used in 1610, the year the King James Bible was completed. The meanings of words have changed over time. So, while the King James Bible was a good translation in 1620, it is not a good translation for today's English speakers. One amazing fact about the original texts is that there are very few copying errors among the many different copies that have been found. The Bible is also the ancient document with the largest number of extant copies found that were made before 200 AD.
