The biggest camps had about 16,000 inmates at any one time. As people were worked to death they were replaced by new arrivals. At the extermination camps, such as Treblinka II and the Birkenau section of Auschwitz, most newly arrived Jews were gassed as quickly as possible after arrival. From about mid to late 1944 onwards many prisoners were moved from the camps in Poland to camps in Germany itself, which then experienced a huge increase in numbers.

95 % --- Please see the list in the link. It gives the numbers (not the percentages) in the final column. There were huge variations. In the extermination camps the percentage was well above 95%, and in ordinary concentration camps it was much lower. For example, at Belzec - an extermination camp - 434,508 Jews and an unknown number of gypsies was slaughtered, and there are only two (!) known to have survived. At Dachau, an ordinary concentration camp, about 80% of those who entered the camp, emerged alive. However, this does not necessarily mean that they survived, as some were sent to other camps, where they perished.

It depended on the ghetto and the year. The Nazis kept on forcing more and more Jews into the ghettos. By early 1942 in the Warsaw Ghetto there were ten (!) people to each room.

The number varied from camp to camp and also over time. A really big camp often had 16,000 or more inmates, small camp a few hundred.
