Not that many: 6 '(very) probable' cases since 1980. In 4 of them the guilty pilot had shut his co-pilot out of the cockpit. The 6 cases include the recent loss of the Malaysian Airlines flight that has all the signs of one of the pilots deliberately shutting out the other pilot, switching off all communications, turning to a part of the Indian ocean not covered by radar systems and crashing the plane there. But we may never be sure as the plane and its black boxes may never be found. And of course there is the very recent German Wings-flight.
