Two out of five Space Shuttles: Challager and Columbia, were the only 2 shuttles to be lost, challager exploded 72 seconds after takeoff while Columbia burned up in the atmosphere.

The space shuttles lost to accidents were the Challenger, which disintegrated in 1986 shortly after launch, and the Columbia, which broke apart during re-entry in 2003. Both accidents resulted in the loss of all crew members aboard.
The Columbia supercomputer helped NASA guide the space shuttle. This supercomputer was named after the Columbia crew lost in an explosion in 2003 at lift off.
The first space shuttle to explode during liftoff was the space shuttle Challengerin 1986. The second shuttle disaster occured on the 1st of Febuary 2003 to space shuttle Columbia. In boh disasters, 7 astronauts lost their lives.
There were six space shuttles built between the late 70s and early 1990s. Their names are: Enterprise, Columbia, Challenger, Discovery,Atlantis and Endeavor. The Enterprise flew several times on top of a modified Boeing 747 Jumbo Jet, and did glidding tests in the 70s, but never went into space. This tests lead to discovering problems with heat tiles falling off that were resolved in later shuttles. Both the Challenger and the Columbia were lost in accidents, the Challenger as it took off from Florida, the Columbia as it re-entered the atmosphere.
The reason being is that N.A.S.A does not want to lose millions and billions of dollars in shuttles or crafts so they put computers that can be tracked and remotely controlled so people do not risk their lives in space and so the shuttles/crafts don't get lost in space.

Another shuttle exists, Enterprise (the first one), but it is not space worthy and was designed for glide tests and fit checks. it has, however, been flown by astronauts, so it deserves mention.
