First you get your undergrad; that is typically 4 years. Following this you get a masters that is typically 2 years. A PhD is granted when you demonstrate a complete mastery of a topic, it can be given quickly if you are very bright or never if you don't ever demonstrate the level of understanding required by your department.

It's not a matter of going to university for a set number of years but the understanding and mastering of the subject.

It varies; normally somewhere between 3 and 7 years of post-graduate study (that is, after receiving a Bachelor's degree) in the US. A Ph.D. differs from other degrees in that it's not "automatic" after completing a particular course of study; the Ph.D. candidate must write a dissertation or thesis... a book detailing his or her particular researches, and setting out some fact that had not been previously known (at least, that's how it works in the sciences; I really don't know exactly what a person has to do to get a Ph.D. in English, but I assume it's something similar) and then defend that research to a panel of advisors, satisfactorily answering their questions and criticisms of the work.

It really depends on the type of PhD, the course load taken each semester, whether a full-time or part-time student, the dissertation and the defence thereof etc. However, typically the PhD will take approximately four years beyond the masters degree.

To get your PhD in Canada requires:

1) A 4 year undergraduate university degree
2) A 2-3 year master's degree
3) A 3-5 year PhD

Some students will begin a masters degree and after 1 or 2 years, instead of completing the masters, convert it into a PhD. But, they will still need to complete another 3-5 years before achieving their PhD.
