A cabin filter needs to be changed at least once a year. A cabin filter filters the air that enters the cabin of your vehicle. So all those impurities in the air outside your car get collected by this cabin filter and accumulate. So if your cabin filter is dirty then you're basically breathing in dirty air and could eventually be harmful.
The cabin air filter can not cause a rough idle. It only filters the air for the ac/heater.
