You have to manage and lead a team including vet techs, vet assistants, kennel personnel, possibly a groomer plus office staff such as a receptionist. Very few medical cases are "classic", so you are often working on imperfect information to figure out what to do and in many cases may be forced to figure it out as you go along. Financial concerns are front and center for many clients, as well as for your business.
