I have a 1998. same problem. I tried drilling and extracting, no avail. It looks like both of us will have to buy a window mount antenna......
. There is a bolt on the bottom of the antenna mount behind the wheel well. You can eathear take your whole wheel well out or you can go under the truck and move the bottom of the wheel well out and remove the bottom bolt and fish it throu. You have to undo one screw on the bottom of the wheel well so you can bend it enouf to get it out. You also have to undo the nut that is at the top of the antenna so that you can take the antenna mast out of top of the top of the fender, you have to be carefull thou you can break the plastic rod that connects the mast to the motor. Good luck I just got done removing mine.

If it has broken off, or simply is not working is a different story. On mine, the antenna snapped off, and you can simply file down the base of the antenna to a smooth edge and hit it back on. Otherwise, try to get underneath the antenna with a flat-head screw driver, or a chisel, and gently bump it up. It should come up after a while. Also, if your antenna seems fine but isn't working, just check the antenna's plug. You can see if its connected from the engine compartment. My radio is original, and wasn't working, and I found that the plug had been disconnected. After I plugged it back in, it worked just fine.
Check to see if any of the " clamps " holding the metal heat shields to the exhaust pipe have snapped ( on my 1995 Ford Explorer XLT I have replaced 5 of the 6 clamps with worm and gear clamps over the years ) Each time one of those narrow little metal bands breaks I would get a tinny rattle from the exhaust that I would notice most at idle
