There were a lot of monarchies, but there were also other types of government. The Republic of Venice dates to the 8th century, but much older republics existed, and people of the Middle Ages all had the example of the Roman Republic to look to for the formation of a government.
The monarchies were often very different from each other. Some were feudal, others were not. Some were hereditary, others had elected monarchs. They were so different that they really have to be described as different types of government.

I think the factors that made the manor the center of the European economy during the Middle Ages were a weak central government and feudalism because the knight that protected the lords got land around the lord's house which created the manor.
