It varies from state to state, but the accepted width of an average car lane is anywhere from nine to fifteen feet. Fifteen feet average on the highway, twelve or eleven feet in urban areas, and, in rural areas, can vary from seven feet to nine feet. Some states have revised statutes (Kentucky's was put in effect 7/2004) which restrict new roads (Kentucky's-fifteen feet minimum). Again, it varies from state to state, probably varies overseas as well.

Who is at the fault when the car from the inside lane go right and hit the car in the outside lane going straight?

With only the information available, it would seem apparent that the car which was originally in the inside lane would be at fault. Vehicles already in a lane of travel have the right-of-way within that lane, and any traffic wishing to merge into that lane is required to yield to traffic already in that lane.

Who is at fault when the left lane merges into the right and the car in the left is to long and has no room to enter the right lane the car in the left hits the car in the right lane?

the car changing lanes, so the left. By Yisoonshin - "I thought that the car on the right would be at fault, because since the left lane is merging, the right car should go faster or slower to allow room for the car in the left lane. It's not like you can stop right there. But usually the right lane merges into the left, or at least I thought they do. By Perau - nope, that's not how it works. the car on that lane has the rightaway to go at whatever speed they like. The car on the left has to wait and make sure it is clear for them to merge.
