Sounds like coolant leak internally, bad head gaskets, cracked head, bad intake gaskets. If leaking into a cyl or more that cyl wont be firing right so raw gas can make it through.
Sounds like a severely cracked head to me. Did you overheat the engine???? Is your coolant level in the radiator going down? I had the same problem with my 94 Cougar 4.6L. I had to replace the ECU. Hard to track down, good luck!!

Could be a diesel? If not then replace your oxygen sensor on your exhaust. A bad oxygen sensor causes the exhaust smoke to be dark in color.
