Engagement means that the widest part of the baby's head has entered the pelvis prior to labour. With first babies this may occur at around 36 weeks with second and subsequesnt babies it may not occur until labour starts. Occasionally a first baby may not engage and the mother will be carefully monitoredas there may be a small pelvis or some other probelm. However usually there is no apparent reason and labour and delivery is quite normal. PERSONAL NOTE As a midwife I once cared for a woman whose baby was not only not engaged at term but was a different way round at every examination so she was staing in hospital. At the end of one afternoon her husband came and asked if I could examine her as she had ben having contractions for over an hour. When I examined her the head was not only engaged but I could hardly feel it. She was rapidly transferred to the labour suite and delivered about one hour later. She must have just had a very relaxed but very efficient uterus.
