The person would have to have a valid license, and this would be in any state. It would also be unwise to let one drive your vehicle without the proper documents. You would be in as much trouble as the other.

Zero. You can't get a medical license without a medical degree. One hundred years ago or more, the erstwhile doctor would work for someone who practiced medicine, much as lawyers learned how to become lawyers. However, the information that doctors must learn has become more formalized as well as there being a need for the community to license practitioners.
