The same way you talked to her 5 weeks ago! Ask her how she's been since you've last spoken, tell her how you've been, a natural conversation is surely to evolve from that. I would say I'm sorry for not talking to you for so long.
You could contact her by phone, email or if you want to talk to her in person and say Hi - its been awhile and I way thinking of you and wondering how you been doing. You will be able to tell from there is she wishes to talk to you or not.

Then perhaps;
Hey! havn't spoken in ages! how you been? and hopefully the conversation will start from that.
