The same way you talked to her 5 weeks ago! Ask her how she's been since you've last spoken, tell her how you've been, a natural conversation is surely to evolve from that. I would say I'm sorry for not talking to you for so long.

∙ 16y agoYou could contact her by phone, email or if you want to talk to her in person and say Hi - its been awhile and I way thinking of you and wondering how you been doing. You will be able to tell from there is she wishes to talk to you or not.

You could suddenly surprise her. You might also talk about a subject you used to talk with her. If she hasn't talked to you yet, most of the times she IS waiting for you to make the "first" move again. You can send her a text if you don't feel like you're ready to talk to her through the phone. If you haven't talked to her in a long time, it would be awkward to text her, before talking to her. Or because you know she likes you, you can always ask her to a second date. Hoped that helped. :)
